A well-established and leading healthcare company is seeking to hire a dedicated and compassionate individual to join the team. You will be providing personalised, high-quality, and cost-effective care to patients, supporting them through treatments and follow-up.

Job Responsibilities:

• Support the nursing team in delivering personalised, efficient, and quality patient care.

• Monitor and report patient conditions, escalating any unusual findings to senior staff.

• Assist in implementing and coordinating discharge and post-discharge care plans.

• Conduct follow-up calls and home or agency visits to ensure continuity of community care.

• Educate patients and caregivers on health management, self-care, and holistic wellbeing.

• Coordinate and schedule home visits for new referrals as required.

Job Requirements:

• Min. Diploma in Nursing

• Registered with Singapore Nursing Board

• Basic Cardiac Life Support (BCLS) + AED certification
