Clinical Duties

1. Provide quality and professional Physiotherapy services in assigned caseloads for inpatient, outpatient and home cases.

2. Provide coverage of service to assigned ward(s).

3. Implement and document the following:

   -  initial assessment

   -  reassessments every 6 monthly for maintenance and 3 monthly for active rehabilitation

   -  Individualised rehabilitation goals

   -  weekly review rehab plan

   -  treatment updates

   -  discharge planning

4. Responsible for follow-up of patients.

5. Sit in case conferences, team meetings and family meetings

6. Comply to hospital clinical guideline with sound clinical reasoning

7. Carry out proper rehab case management round with OT/PT

8. Carry out proper hand over for leave coverage

9. Provide holistic rehab input in case discussion

10. Carry out proper and sufficient safety measures to protect our patient during rehab. Risk assessment of clinical setting.

11. Communicate to patient / family member to enhance rehab care, skill application and promote therapeutic carry over after rehab session

12. Responsible for the safe use of all equipment and report all equipment defects, accidents and complaints to the appropriate authority for further resolution if required.

13. Engage clients / families / carers in the treatment, review and rehabilitation of clients.

14. Provide caregiver training to family members or carers safely and professionally when required.

15. Use evidence-based standardised assessment and treatments to assess, plan, implement and evaluate interventions.

16. Monitor, evaluate and review physiotherapy treatment and interventions within the multi-disciplinary framework and modify as necessary.

Administrative Duties

1. Responsible for daily billing.

2. Organize caseload coverage and work distribution.

3. Supervise rehab assistants. (PT is responsible for patient’s safety)

4. Document and report incident when necessary.

5. Maintain a neat and tidy workplace

6. Advocate Allium Healthcare’s mission, vision and goals

7. Ensure appropriate, accurate and up-to-date assessment records and client’s treatment plan are maintained in the infocare systems in accordance with the organization policies.

8. Supervise and conduct therapeutic programmes for clients during outreach programs

9. Assist in collecting/maintaining statistical information needed for regular reports

10. Work within the Enhance Nursing Home Standards Guidelines.

11. Participate in continuing professional development and keep up to date with current practice, new techniques and technologies available for treating clients.

12. Participate in the in-service training, lectures and education programmes to staff, other disciplines, students and general public.

13. Participate in health promotion and health education initiatives within the community.

14. Maintain clients’ confidentiality as prescribed by the Personal Data Protection Act (PDPA) and the organization’s policies and protocols.

Education and Training

1. Participate in departmental in-service training.

2. Involve in in-service education for other professionals and students.

3. Teach and supervise PT students in their clinical placements.

4. Give talks/ lectures when necessary.

5. Involve in research activities.

Required Education and Experience

1. Master/Bachelor/Diploma in Physiotherapy, recognized by the Singapore Allied Health Professional Council (AHPC).

2. Registered with AHPC and holding valid practicing certificate from AHPC.

3. BCLS + AED Certificate

4. Relevant Post Graduate qualification is desirable

5. 2-3 years’ experience relevant experience in hospital or nursing home setting

6. Relevant community and aged care experience would be desirable.
