Responsibilities:
Guide the Care Coordination team to assist patients and families in accessing the necessary services and programs for optimal health outcomes.
Conduct post-discharge follow-ups through phone calls and home visits, ensuring that patients and caregivers can effectively manage care.
Streamline community services and provide educational resources to caregivers, including assistance with end-of-life care planning.
Oversee data entry, management, and analysis to support coordination efforts.
Collaborate with partners in Acute Care and Intermediate and Long-Term Care (ILTC) settings to facilitate ongoing support for patients post-discharge, aimed at reducing hospital readmissions.
Design and implement programs and services tailored to meet community patient and caregiver needs.
Provide clinical supervision and oversee staff performance, ensuring high standards of care.
Assess and plan for the professional development and training of team members to maintain clinical competence.
Evaluate and refine work processes in collaboration with internal and external stakeholders to enhance care quality and efficiency.
Participate in or lead various hospital initiatives (e.g., quality improvement, LEAN projects, or research) that focus on elevating the standards of clinical care.
Requirements:
Professional qualification in Nursing, Social Work, Allied Health, or related clinical disciplines relevant to the position.
A minimum of 8 years of experience in Care Coordination, Case Management, or as a Community Nurse within a healthcare setting.
At least 3 to 5 years of leadership experience in managing a team.
