Key Responsibilities

• Deliver quality geriatric and dementia care in accordance with approved nursing standards.

• Conduct pre-assessments and coordinate enrolment of clients referred by AIC, ensuring compliance with SOPs.

• Develop, implement, and review holistic care plans in consultation with the multidisciplinary team.

• Administer medication, perform wound care, and respond to emergencies (BCLS, CPR, AED, First Aid).

• Provide health education, counselling, and care coordination for clients and families.

• Supervise and coach Nursing Aides and Care Assistants, ensuring competency and adherence to care standards.

• Lead infection control initiatives, clinical audits, and service quality improvement projects.

• Support staff development through orientation, training, and in-service education.

• Ensure compliance with MOH, ENHS, and clinical governance standards.

• Maintain accurate documentation and liaise with healthcare partners to ensure continuity of care.

Who You Are

• Registered Nurse (SNB) with valid practising certificate.

• Passionate about eldercare and dementia care.

• Possess strong clinical judgment, leadership, and communication skills.

• Enjoy mentoring junior nurses and care staff in a collaborative team setting.

• Prior experience in community, home care, or senior care settings preferred.
