Community Nurse (Daycare/West/Case Management) (ID: 672257) - #1117337

Responsibilities:

• Deliver quality geriatric and dementia nursing care, including pre-assessments, care planning, medication administration, wound care, and emergency response (CPR, AED, first aid).

• Collaborate with healthcare teams to assess, monitor, and review clients’ conditions, ensuring care plans are effectively implemented and adjusted as needed.

• Lead, supervise, and support nursing and care staff to maintain high standards of clinical and service quality.

• Provide health education, counselling, and promote wellness activities for clients and families.

• Participate in quality improvement projects, infection control initiatives, and training of care staff to drive continuous improvement.

Requirements: 

• Diploma or Degree in Nursing.

• Registered nurse with Singapore Nursing Board.

• At least 3 years' experience nursing experience in Singapore.

• Possess knowledge and understanding of community care practices.

• Comfortable to travel for home visit.
