Job Summary:
The Senior Physiotherapist (SPT) provides physiotherapy services to optimise clients’ functional abilities, independence, and quality of life in home care setting. The role includes developing and implementing care plans, guiding clients and caregivers in rehabilitation strategies, supervising junior staff and students, and contributing to service development and quality improvement.
Principal Responsibilities and Duties:
Clinical Responsibilities
Render clinical services to clients through assessment of client’s functional status, performance deficits and care resources.
Offer strategies for clients to manage their physical rehabilitation needs while reducing the risk of injury and further functional decline.
Formulate and implement care plan to facilitate maximal independence and prevent disability.
Prescribe, adapt and train clients and caregivers on the use of rehabilitative equipment and assistive devices/applicant.
Monitor client’s progress and maintain timely and accurate documentation of patients’ condition and care and outcomes.
Liaise with other internal or external care providers, to plan, develop and implement patient care in discussion with patients, care providers and their caregiver.
Connect clients with community resources and financial assistance when assessed, as necessary.
Conduct caregiver training programme to clients / caregivers and evaluate effectiveness of training by assessing improvement in care and caregiver/s coping capacity.
Initiate and oversee discharge planning from admission to ensure smooth transition of care.
Provide clinical supervision, mentoring, and competency assessment for junior physiotherapists and students.
Keep abreast of current professional practices by networking with relevant bodies and participating in professional education activities.
Support Operations Manager in service analysis, audits, and development of community fitness or rehabilitation programmes.
Co-lead professional development activities and projects (e.g training, quality improvement project, research or any new initiatives as assigned
Coach junior staff and therapy assistants to build capability and improve service standards.
Recommend services or care arrangements as appropriate, ensuring accurate documentation.
Assist in recruitment processes for physiotherapy or therapy assistant positions.
Monitor and achieve assigned KPIs, supporting initiatives for work efficiency and service quality.
Essential Skills and Qualifications
Bachelor’s degree in Physiotherapy from a recognised institution; eligible for registration with AHPC (Singapore).
Preferably with experience in geriatric, community, or rehabilitation settings, including mentoring or supervising others.
Strong clinical reasoning, assessment, and care planning skills.
Strong communication and interpersonal abilities for working with clients, caregivers, and multidisciplinary teams.
Ability to coach and supervise junior therapists and students.
