Job Responsibilities:  

• Assist patients and families with intake interviews, financial assessments, and referrals to support discharge planning.

• Support Medical Social Workers with case management, translation, patient escort, and home visits.

• Contribute to process and quality improvement initiatives.

• Coordinate with internal departments and external agencies to enhance patient care and experience.

• Provide administrative and front desk support for daily operations.

• Assist in departmental programmes and projects.

Requirement:

• Diploma in Social Work

• A good team player, meticulous

• Able to work independently

• Ability to multitask and handle stress
