Job Description

As a Care Coordinator, you will play an important role in supporting patients, caregivers, and healthcare teams. Your responsibilities include:

• Coordinating and providing patient care activities with the use of IT systems

• Building strong relationships with patients, caregivers, and healthcare professionals

• Performing basic parameter measurements and assisting in preventive health screenings and vaccinations

• Providing basic health education to improve lifestyle management, social engagement, and physical well-being

• Encouraging the use of health monitoring devices (e.g. blood pressure, glucose, weight monitoring)

• Empowering patients through technology and guiding them to available healthcare resources

Requirements

• Diploma and above

• Healthcare work experience will be advantageous

• Strong communication skills (verbal & written) in English and a local language/dialect

• Proficient in Microsoft Office (Word, Excel, PowerPoint)

• IT-savvy and able to navigate digital platforms with ease

• Pleasant personality with good interpersonal skills and a positive attitude

• Self-motivated, team player, and genuinely interested in patient care
