As a Home Care Nurse, you will be responsible for the following duties:

• To perform home visits for the palliative care patients.

• Perform nursing assessment on patients.

• To initiate nursing care plan and implement new measures.

• Working closely with the Medical Director or Doctors on the case.

• To educate caregivers/families on basic nursing procedures.

• Providing emotional and bereavement support

Requirements:

• Minimum Diploma or Degree in Nursing

• SNB Registered nurses

• Ability to work independently

• At least 1 – 2 years of experience in homecare services.

• Able to work in fast-pace environment
