Job Scope

• Engage patients referred by the healthcare team to participate in community-based activities, either in person or through follow-up phone calls.

• Introduce patients to a range of available community activities, assist them in selecting suitable options, and support their registration process.

• Educate and motivate patients on the value of community participation in improving health and well-being.

• Follow up with patients to check on their attendance and participation in activities.

• Assist with basic navigation support and respond to general queries from patients.

• Facilitate the completion of patient questionnaires when needed.

• Coordinate with community partners to gather and update schedules of ongoing programs.

• Submit sign-up records to partners for tracking and retrieve patient attendance information.

• Maintain accurate records of all patient engagements and referrals.

• Provide timely updates to the clinical team on each patient’s community involvement.

Requirement

• Minimum GCE O Level and above
