Snr / Staff Nurse (East, Daycare/ Eldercare, Office Hours) - HWX - #1106555

Responsibilities:

• Implement nursing interventions as outlined in the care plan and carry out nursing procedures according to the care needs of the client;

• Provide training and supervision to care staff;

• Co-ordinate with other team members and update them on the changes in the client's condition and needs

• Prepare clinical documentation, monitor outcomes of services, and perform escalation of cases if there is a need for more acute management of clients;

• Provide ongoing support and education to the caregiver, client, and family;

• Carry out other responsibilities and duties as assigned from time to time, including coverage of other Staff Nurse in other Senior Care facilities if necessary.

Requirements:

• Min. Diploma/Degree in Nursing from a recognized tertiary institution

• Registered with Singapore Nursing Board

• Possess min. 5 years prior nursing experience in preferrably Home-based Nursing, Nursing Homes, Acute Hosp / Comm Hosp (Geriatric / General / Cardiac / Ortho / Medical / Neurology)

• Excellent interpersonal relationships, in collaboration with various partners in the healthcare
