As a Care Coordinator (Primary Care setting), you will be responsible for the following duties:

• Empower patients and caregivers through tech and education.

• Build strong ties with care teams and the community.

• Support preventive screenings and vaccinations.

• Use IT systems to track care and outcomes.

• Help reduce hospital readmissions and escalations.

• Address social determinants of health.

• Conduct basic health checks and promote home monitoring.
