As a Care Coordinator (Primary Care setting), you will be responsible for the following duties:
Empower patients and caregivers through tech and education.
Build strong ties with care teams and the community.
Support preventive screenings and vaccinations.
Use IT systems to track care and outcomes.
Help reduce hospital readmissions and escalations.
Address social determinants of health.
Conduct basic health checks and promote home monitoring.
Provide health advice and social prescribing.
Requirements
Possess a Diploma in Health Management & Promotion or equivalent.
