Community Outreach Team (CREST), serves as a community safety network for people with or at-risk of mental health conditions, including dementia, and their caregivers. CREST conducts outreach events to increase mental health awareness and targeted case finding to identify those with mental health needs.

CREST works with clients aged 18 years and above.

Reporting to the Group Supervisor, the Programme Coordinator/Social Work Associate works closely with the team to serve clients and caregivers.

  • Assist to plan, coordinate and execute outreach activities to educate the public, seniors and partner stakeholders on dementia and mental health awareness.

  • Assessment and review information and referral service and monitoring of clients with mental health needs, and dementia.

  • Networking with community partners.

  • Administrative duties

  • Other work projects assigned by supervisor/management.

Candidate Requirements

Technical Competencies:

  • Diploma in Social Work or Psychology or Counselling required.

  • IT literate.

Core Competencies:

  • Good communication and social skills to work with different people.

  • Pleasant personality.

  • Team player, yet independent to carry out most functions.

  • Strong interest in empowering and preserving the dignity of people at-risk or with mental health conditions including dementia.

Preferred achievements / characteristics:

  • Able to converse in dialects preferred.
