Job Description:
Initiate contact with adolescent patients visiting the clinic.
Accompany the Medical Social Worker on field visits as needed.
Manage email and phone inquiries related to the adolescent mental health program.
Maintain program data.
Schedule and coordinate meetings.
Assist the Medical Social Worker with case management tasks and other patient profiles/projects as required.
Requirements:
Diploma holder
At least 1 year of relevant working experience
Experience in working with adolescents will be an advantage
