As a Care Manager (Nursing) , you will be responsible for the following duties:

• Handle patients who have chronic conditions like diabetes or hypertension to help them manage their disease, including understanding their condition, developing self-management skills, and monitoring their progress. 

• Utilize digital tools, such as patient portals or apps, to track patient well-being and facilitate communication. 

• Build trusting relationships with patients and their families, providing ongoing support and guidance. 

• Provide initial assessments and counselling for mental health concerns. 

• Coordinate care with other members of the healthcare team, including doctors, nurses, and allied health professionals, to ensure a holistic approach to patient care.

• Provide education on lifestyle changes, medication adherence, and other aspects of managing their condition. 

Requirements:

• At least a Degree in Nursing

• Possess Advanced Diploma will be advantageous

• Possess minimum 5 years of supervisory / managerial experience

• Must be registered with Singapore Nursing Board (SNB)

• Excellent interpersonal and communication skills

• Great leadership and management skills
