Counsellor / Social Worker (Advance Care Planning) - #1099174

Job Scope:

• Engage all suitable referrals in Advance Care Planning discussion and facilitate the completion of Advance Care Plans for patients.
• Responsible for effective communication of patients’ Advance Care Plans to health care professionals and other stakeholders.
• Follow up and review outcome of Advance Care Planning, including honouring of care and treatment preferences. 
• Follow up on concerns raised during Advance Care Planning discussion, by providing feedback to the healthcare teams and other stakeholders.
• Work in partnership with stakeholders to raise awareness of Advance Care Planning.
• Conduct and participate in Advance Care Planning advocacy and facilitator training.
• Ad hoc duties as assigned 

Job Requirements:

• Degree in Nursing, Social Work, Psychology, Allied Health or equivalent 
• Good interpersonal and communication skills
• Proficient in using MS office
• Knowledge in geriatric, patient care and community skills will be an advantage
