Renal Coordinator - #1098239

The Renal Coordinator is responsible in educating mild chronic renal failure patients and end stage renal failure patients in self-care and teaching patients at risk of developing renal failure on retardation or prevention of the disease.

The role involves:

1. Providing patient education for chronic kidney disease patients
2. Presenting treatment options for moderate to end stage renal failure patients
3. Providing financial counselling for new dialysis patients or patients who are going to initiate dialysis
4. Providing counselling service
5. Patient case management
6. Coordinating of dialysis plan
7. Monitoring of Ambulatory Blood Pressure
8. Maintaining and updating of CKD (Chronic Kidney Disease) and ESRD (End Stage Renal Disease) Registry

Job Requirements
