Care Associate (Active Aging Centre (Care) / Active Aging Centre) - #1098173

Purpose

Being an important member of MDT to deliver nursing care and assistance in programme/activities to SSC Clients.

Area of Accountability

1) Assist RN in Delivering nursing and custodial care and assist PT/OT in programme/activities implementation to Day care clients.

• Assist in consulting and coordinating with health care team to assess, plan, implement and evaluate client’s condition.
• Assist in developing, reviewing, and updating ICP with multiple disciplinary approach and Person- Centred Care approach.
• Support and help the Nursing staff in providing the Clients with custodial care such as medication assistance toileting, Changing of Diaper, assist clients in eating.
• Facilitate and/or assist in group/individualised exercise programmes and activities to SCC clients.
• Provide safe transport escort services.
• Work together with other MDT members for behaviour issues addressed from Dementia Day Care clients and any other Day Care Clients.

2) Compliance with approved standards and service requirements in proper documentation for Day Care clients

• Coordinate client’s visit schedule, record attendance and activity attendance.
• Perform proper documentation of patients’ care/progress notes in a timely and accurate manner.

3) Maintenance of Centre equipment and inventory, clinical items according to service requirement and company rule, and ensure proper recording of inventory.

4) Quality Assurance and others

• Involve in quality procedures and guidelines and policy reviews when required.
• Apply compliance to the Company’s quality procedures, guidelines, and policy. Monitor and report non-compliance and implement necessary changes resulting from quality assurance findings in consultation with Physiotherapist or Occupational Therapist
• Undertake responsibility and duties as assigned by RO.

Education Level

• At Least “O” Level

Any other knowledge required

• Good communication and Interpersonal skills
• Passionate in taking care Elderly.
• to transfer and/or lift client with proper manner.
• Experience in healthcare/community care setting would be an advantage.
