Job Title: Mental Health Staff Nurse, Care Coordinator
Salary Range: $4,000 – $5,000 per month
Work Schedule: 5 Days, Office Hours
Employment Type: Full-Time

Key Responsibilities:

• Conduct home visits to assess client needs and provide psychoeducation and support.

• Coordinate services across healthcare and community systems to meet clients' medical, psychological, social, and vocational needs.

• Work collaboratively in a multidisciplinary team with doctors, nurses, and allied health professionals.

• Contribute psychological insights during team discussions and care planning.

• Deliver training and education to caregivers and team members when necessary.

• Facilitate support groups for clients and caregivers.

• Undertake assigned projects and departmental duties.

Requirements:

• Registered Nurse with the Singapore Nursing Board (SNB).

• Advanced Diploma in Mental Health Nursing or related fields (e.g., Social Work, Psychology, Counseling).

• At least 3 to 5 years of experience in psychiatric or mental health settings (e.g., community psychiatric clinics or IMH).
