This role will support Manager, Centre-Based Care, to ensure smooth and effective execution of new care pilots, services and programmes aimed at enhancing client care quality and community care integration.
Working Location: Woodlands Care Home
Provide administrative and operational support for new care pilots, services, and programmes for Centre-Based Care.
Publicity & Engagement
Support publicity and engagement efforts, including engagement with both internal and external stakeholders, as required, to drive the take-up rate for the new care pilots, services and programmes
Project Management & Administration
Support the planning and execution of new care pilots, services and programme
Provide administrative support for the projects including referral management, data entry, and upkeeping of administrative documentation including project database
Collect feedback for evaluation of service and care model
Day-to-Day Operations
Oversee day-to-day operations of the pilot by planning and coordinating the scheduling of services, including the coordination / communication with NOK
• Work with SCC/AAC team to coordinate the deployment of care staff to ensure smooth and optimal delivery of services as required
• Work with AAC team to recruit micro-jobbers for pilots, and coordinate the activation of micro-jobbers as required
• Work with Finance to ensure accurate billing for the pilots
• Ensure smooth roll-out and operations by supporting referral management e.g. pre-admission intake, basic financial counselling, follow-up with service admission and discharge, and conducting of home assessments, as required for the pilot roll-out
• Escalate cases requiring financial assistance to Medical Social Worker as necessary
Other Tasks and Responsibilities
Any other tasks and responsibilities as assigned by Manager/Supervisor
Requirement
Degree in relevant field with at least 1 to 3 years of relevant healthcare industry experience
Proficient in Microsoft Office
Good communications and people engagement skills with ability to communicate with clients in their local dialect languages
Good organisation skill, with ability to multi-task and eye for details
Takes initiative to identify, plan, and implement ways to optimise work processes.
